It could be an honest mistake but its one that seems to happen quite frequently. Its not the first time I have read articles like that one.

I read one years ago where a bank did something similar. The person died, but they kept sending statements and all sorts, and the executor of the wíll had to fight with the bank for ages to get them to stop. They wanted to speak to the deceased person. I think too business people get so wound up in life and stress that they lose a few marbles along the way. They lose touch with reality at some level or the synapse stop firing... :Scratch Head:
